29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:     At a jobsite in Rockton, Illinois employees were not provided a safe means of access to the ladder where a break in elevation exceeded 19 inches.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.  At a jobsite in Rockton, Illinois the side rails of an extension ladder did not extend a minimum of 3 feet above the landing surface that was being accessed.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.     At a jobsite in Rockton, Illinois employees working on the roof of a newly constructed commercial  building installing decking approximately 17 feet above grade were not using any method of fall protection.     The Schmidt Steel Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.760(a)(1), which was contained in OSHA inspection number 191925, citation number 1, item number 1 and was affirmed as a final order on April 11, 2012, with respect to a workplace located at 1528 Brook Drive, Downers Grove, Illinois.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
