29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Employees were exposed to the hazard of falling approximately 16 feet from the edge of the roof deck while engaged in roofing activities without the use of fall protection or a safety monitoring system. Waukegan Roofing Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(10) which was contained in OSHA inspection number 314038183, citation 1, item 1, and was affirmed as a final order on October 13, 2010, with respect to a worksite located at 4305 Weaver Parkway, Warrenville, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.502(f)(2)(ii): Warning line systems were not rigged and supported in such a way that its lowest point (including sag) was not less than 34 inches from the walking/working surface: a) A warning line system positioned along the east edge of the roof deck was not rigged and supported to prevent sagging below 34 inches where employees were exposed to a fall hazard of approximately 16 feet during the performance of roofing work activities. Sections of the warning line system sagged to the roof deck. Waukegan Roofing Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502 (f)(2)(ii) which was contained in OSHA inspection number 312728322, citation 1, item 2, and was affirmed as a final order on September 3, 2009, with respect to a worksite located at 815 Church, Elmhurst, Illinois. No abatement certification or documentation required for this violation.
