29 CFR 1910.141(d)(3)(iv): Showers were not provided with hot and cold water feeding a common discharge line: (a) Upper Level - Employees were performing asbestos remediation activities inside a containment that was equipped with a shower that only had cold water, on or about May 20, 2013. ASI DBA as Asbestos Specialists Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.141(d)(3)(iv), which was contained in OSHA inspection number 316302702, citation number 1, item number 3 and was affirmed as a final order on June 22, 2012, with respect to a workplace located at 1346 Park Road, NW, Washington, DC 20010. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use: (a) Upper Level - Employees performing asbestos remediation activities were required to wear half-face respirators and the employer did not have a respiratory protection program with worksite-specific procedures, on or about May 20, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
