29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.   (a)  Construction Site - On or about June 5, 2013 employees were exposed to falling onto wood boards and wood boards with protruding nails. Several wood boards were directly under a ladder where an employee exited the ladder.

29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment shall be guarded if such parts are exposed to contact by employees or otherwise create a hazard. Guarding shall meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), Safety Code for Mechanical Power-Transmission Apparatus.   (a)   Back of House - On or about June 5, 2013 employees were exposed to an amputation hazard while using a Jenny 8-gallon Gas Wheelbarrow Air Compressor with belts and pulleys unguarded.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.     (a)    Front of House - On or about June 5, 2013 an employee was exposed to fall hazard while using an aluminum extension ladder that was not extended 3 feet above landing or tied to the building.          (b)   Right Side Roof - On or about June 5, 2013 an employee was exposed to fall hazard while using an aluminum extension ladder that was not extended 3 feet above landing or tied to the building.
