29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  On June 6, 2013, at a jobsite located at 923 W Vest - Marshall, MO., Employees were exposed to serious struck-by injuries, in that employees working in the immediate areas below an elevated aerial lift were not wearing protective helmets.    Abatement certification for this violation is not required. Abated during the inspection.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    (a)  On June 6, 2013, at a jobsite located at 923 W Vest - Marshall, MO., Employees were exposed to serious fall hazard injuries, in that an observed employee engaged in Exterior Installation Activities from an elevated Aerial Lift was not wearing fall protection equipment that was attached to the boom or basket. The employee was exposed to fall hazards from approx. 13 ft up approx. 20 ft to lower levels.   Abatement certification for this violation is not required. Abated during the inspection.
