29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    (a)  The employer did not ensure that employees who accessed the mezzanine level to change out soap totes or perform minor servicing work were protected from falling to the lower level by the use of standard guard railings or an equivalent form of fall protection.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)  The employer did not develop and document energy control procedures that were specific to the equipment employees performed maintenance/servicing on, such as but not limited to Yale fork trucks or the air compressor.       YES Equipment & Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.147(c)(4)(i)], which was contained in OSHA inspection number 721501, citation number 1, item number 2 and was affirmed as a final order on March 8, 2013, with respect to a workplace located at W136 N4901 Campbell Drive, Menomonee Falls, Wisconsin.
