29 CFR 1926.453(b)(2)(iv): Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.  On or about June 6, 2013; on the east side of the jobsite, the employer allowed employees to stand on the mid-rail and top rail of an aerial lift basket and exit the aerial lift to access the roof while installing moisture barrier on the exterior of the building.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.   On or about June 6, 2013; on the east side of the jobsite, the employer allowed employees to utilize an aerial lift to install moisture barrier on the exterior of the building without an adequate personal fall arrest system at a height of approx. thirty (30) feet.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  On or about June 6, 2013; on the west side of the jobsite (building 2), the employer allowed employees to perform work while standing on the edge of a roof approx. thirty (30) feet above a lower level without the use of an appropriate fall protection system.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about June 6, 2013; on the west side of the jobsite (building 2), the employer allowed employees to perform work while standing on the edge of a roof approx. thirty (30) feet above a lower level without providing fall protection training to those employees.
