At the jobsite adjacent to the residence on 1429 Mautenne Drive, Ballwin, MO 63011, a Doosan Construction Crawler Excavator, Model: DX 340 LC, Product Identification Number: DHKHECCOS60005243; with a suspended load was not lowered or blocked while not in use. The excavator was used in an excavation approximately ten (10) feet deep, by twenty-five (25) feet wide, by nineteen (19) feet long.

At the jobsite adjacent to the residence on 1429 Mautenne Drive, Ballwin, MO 63011, employees were working in an excavation approximately ten (10) feet deep, by twenty-five (25) feet wide, by nineteen (19) feet long and not protected by an adequate protective system. The employer did not provide adequate protective system to safeguard employees from cave-ins.
