29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) On or about May 16, 2013, at The Timbermen, Inc., employees performing PIT maintenance were exposed to struck-by hazards.  Employees were allowed to use wooden blocks to support a 6,000 pound forklift while performing maintenance and/or servicing activities. The wooden blocks used did not provide the support and stability comparable to jacks and supports required by the forklift manufacturer for vehicle oil changes. A specific lockout/tagout procedure must be developed and utilized when performing vehicle maintenance servicing that includes, but is not limited to identifying, safely installing, and removing the forklift support devices.    b) PET Saw Area-An employee was exposed to a struck-by hazard when observed walking on the equipment of the PET saw to clear out a jam on the belt.  Lockout/tagout procedures were not utilized, on or about October 22, 2013.
