29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentally walk were not guarded by either standard railing with standard toeboard on all exposed sides, or by a floor hole cover of standard strength and construction: On or about June 10, 2013, at the dumping area of the crusher, the expanded metal flooring on the south side of the hoppers had several open holes, 3 to 4 inches in width, that could lead to tripping or falling hazard(s).

29 CFR 1910.23(c)(3): Regardless of height, open sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing system: On or about June 10, 2013, standard railings or equivalent were not in place in several locations in the facility such as, but not limited to the following: 1. The platform walkway leading to or near the jaw crusher, in operation at the platform level, had no guardrails present. 2. The catwalk between the two conveyors leading away from the crushing operation to the grinding area, within 2 to 4 feet from the operating conveyors, had missing guardrails on both sides of the catwalk. 3. The top rails of the walkway, south side of the crushing hopper at or near the top of the sloping to the crusher, were bent down to 36 inches, below standard guardrail top rail minimum of 42 inches. 4. The open doorways by the walkway on the north side of the hoppers for the crusher loading operation, at the top of the sloping to the crusher below, had no guardrails present.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, were not guarded: On or about June 10, 2013, the guarding over the conveyor to press #3 was in disrepair as there were several points open 2 to 4 inches or more, approximately 79 inches above floor level, exposing workers to the moving conveyor system.
