29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews:      On or about June 11, 2013, and the times prior thereto, at or near the worksite located at 523 Kirk Rd in Little Rock, Arkansas, on scaffolding on the west end of the building, guardrails were not provided on the open ends of the cantilever work platform located on the third tier of the scaffold system.  Employees were not protected from falling through the opening.  Employee(s) were exposed to hazard of fall from an elevation.
