29 CFR 1926.100(a): The employer did not ensure that employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were protected by protective helmets. This violation occurred on August 12, 2014, where at least five (5) employee working below employees working on aerial lifts, ladders and roof work above were exposed to being struck by falling materials and tools from approximately 15 feet above.
