29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:   Employees preforming roofing activities on a 5 in 12 pitch roof were exposed to a fall hazard of 25 feet to a lower level. The anchorage point used to tie off to was not capable of supporting at least 5,000 pounds per employee attached.

Employees working from a 5 in 12 pitch roof were exposed to falls of approximately 25 feet to a lower level. Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   Employees working from a 5 in 12 pitch roof were exposed to falls of approximately 25 feet to a lower level.
