29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s): (a) On the roof above the third story at 24 Slater Street, Webster, MA 01570 on or about 4/19/2016 employees performing residential construction (roofing) activities repaired and/or replaced shingles and generated debris but did not use an enclosed chute to dispose of same, resulting in shingle debris striking the ground in and around the work area.
