Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about June 7, 2013 - at the above addressed jobsite, employees conducting roofing operations, approximately eight (8) and thirteen feet nine (13'9") inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.

On or about June 7, 2013 - at the above addressed jobsite, employees performing roofing operations were exposed to falls when the access ladder to the roof did not extend three(3) feet above the upper landing surface.
