29 CFR 1910.26(a)(1): The employer did not ensure that the ladder design was without structural defects or accident hazards such as sharp edges, burrs, and that the metal selected was of sufficient strength to meet the test requirements, and protected against corrosion unless inherently corrosion-resistant: a/ Hackettstown Regional Medical/Hackettstown NJ Site: North Side of Building - Aluminum Extension Ladder had rusted components that are used to secure the ladder footing. On/or about 05/12/2013.

29 CFR 1910.303(f): Each service, feeder and branch circuit, at its disconnecting means or over current device, was not legibly marked to indicate its purpose: a/ Hackettstown Reg. Med. Ctr./Hackettstown NJ Site - Facility Administration Hallway: Electrical Breaker Panels #LPINN #1 and #2 (Including but not limited to) - Where electrical boxes/breakers had typed markings to indicate their character/function, the print was not large enough to be readily seen in the event the equipment needs to be accessed. Observed on/or about 5/12/2013. The Hackettstown Regional Medical Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(f), which was contained in OSHA inspection number 280190, citation number 2, item number 2 and was affirmed as a final order on July 3, 2012, with respect to a workplace located at 651 Willow Grove St., Hackettstown, NJ 07840.
