29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable. a. West Wall/Mill # 1 - On or about June 14, 2013, the ground floor at the end of the working platform was not in dry conditions in that coolant fluid (ecocool syn 7050) from the mill was spilled, thereby exposing employees to slip hazards. b. West Wall/Mill # 1 - On or about June 14, 2013, the floor drainage's metal grating was not properly maintained in that the spaces between were full of dirt and grease (clogged), thereby exposing employees to slip hazards.

29 CFR 1910.22(c): "Covers and guardrails." Covers and/or guardrails shall be provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc. a. West Wall/Mill # 1 - employees were required to walk and work in an area with an open ditch that was not provided with a cover or guardrail. Employees were thereby exposed to slips, trips, and falls.

29 CFR 1910.1048(d)(2): Initial monitoring. The employer shall identify all employees who may be exposed at or above the action level or at or above the STEL and accurately determine the exposure of each employee so identified. a. West Wall/Mill # 1 - On or about June 14, 2013, the employer had not conducted initial monitoring to determine exposure to formaldehyde for employees working around hot manufacturing processess of metal products. Employees were thereby potentially exposed to the hazards of formaldehyde.
