29 CFR 1910.134(d)(1)(iii): The employer shall identify and evaluate the respiratory hazard(s) in the workplace; this evaluation shall include a reasonable estimate of employee exposures to respiratory hazard(s) and an identification of the contaminant's chemical state and physical form. Where the employer cannot identify or reasonably estimate the employee exposure, the employer shall consider the atmosphere to be IDLH.    a. On or about June 06, 2013, at the above located workplace, employees used portable fire extinguishers to extinguish a fire on the thermo molding machine 212. The employer did not evaluate the respiratory hazards the employees were potentially exposed to. Respiratory hazards due to thermal decomposition included, but not limited to aldehydes, alcohols, organic acids, and hydrocarbons.

29 CFR 1910.157(g)(1): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.    a. On or about June 06, 2013, at the above located workplace, employees used portable fire extinguishers to extinguish a fire on the thermo molding machine 212. The employer did not provide an educational program to familiarize all employees with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting.

However, an employer with 10 or fewer employees may communicate the plan orally to employees.  a. On or about June 06, 2013, at the above located workplace, employees used portable fire extinguishers to extinguish a fire on the thermo molding machine 212. A written emergency action plan was not kept in the workplace and made available for review to employees.

On or about June 06, 2013, at the above located workplace, employees used portable fire extinguishers to extinguish a fire on the thermo molding machine 212. The employer did not train all employees to conduct a plant evacuation in case of an emergency such as a fire.
