29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a)  In the forklift battery charging area:  On June 12, 2013 and at times prior to; the employer failed to provide an eye wash station exposing employees to eye injuries from battery acid when checking the forklift batteries.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate an industrial truck:  (a)  In the facility:  On June 12, 2013 and at times prior to; the employer failed to ensure that all employees received training and evaluation in accordance with this paragraph, exposing the operator and employees working in the facility to struck-by and crushing hazards.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:   (a)  On the southwest corner of the building, in the compressed gas storage area:  On June 12, 2013 and at times prior to; the employer failed to separate an oxygen cylinder from fuel gas cylinders such as, but not limited to acetylene and propane exposing employees to burns and smoke and fume inhalation from an oxygen enriched fire.
