29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) Show Room - Platform; On or about September 19, 2018 and at times prior thereto the employer exposed employees to an 11.5 foot fall hazard in that employees were required to remove handrails and work around an unprotected edge in order to retrieve parts that were stored on the platform.

29 CFR 1910.29(b)(13): When guardrail systems are used around holes that serve as points of access (such as ladderways), the guardrail system opening did not meet the requirements (b)(13)(i) or (b)(13)(ii) of this section: a) Show Room - Platform; On or about September 19, 2018 and at times prior thereto the employer exposed employees to an 11.5 foot fall hazards in that employees were required to work on a platform with an open ladder access that was not provided with a swinging gate or so offset that a person could not walk directly into the opening.
