29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about June 14, 2013, an employee was exposed to struck by hazards when not wearing a protective helmet, while handing sheet metal, with sharp edges, to another employee who was working above him on the roof approximately 3 feet above the employees' head.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about June 14, 2013, employee was exposed to falls of greater than 6 feet in height to the concrete level below, who was not protected from falling off the roof, while installing the sheet metal on a roof pitch greater than 4:10. The American Roofing and Metal Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315630855, citation number 1, item number 1 and was affirmed as a final order on 05/07/2012, with respect to a workplace located at 309 Burr St., San Antonio, TX 78209.
