29 CFR 1910.95(g)(6):  At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels.     a).  Gas Propellant Room:  The Bunker Mechanic on Line 2 had a measured noise exposure of 91.8 dBA as an 8 hour time-weighted average on 6/19/2013.  This exposure is 1.28 times the OSHA Noise Permissible Exposure Limit.  The employee's last audiogram was on 11/2/2011.  The employer did not perform annual audiograms for employees in the year 2012.

Propellant Charging Room:  The employer did not have a record which documented that the employees working in this room had received their initial and refresher training in the proper start-up of the propellant charging operation, the routine shut-down of the operation, and the emergency shut-down procedures.      b)  Maintenance of the LPG Systems and Piping:  The employer did not have a record that the maintenance men who perform work on the LPG systems and piping had received training in safe procedures.

Propellant Charging Room:  The employer did not provide shields or barriers to protect employees from being sprayed with pesticide when cans with miscrimps are being pressurized and then rupture with the resulting aerosol being sprayed in the direction of employees.
