29 CFR 1910.95(i)(3): Employees were not given the opportunity to select their hearing protectors from a variety of suitable hearing protectors provided by the employer:    Employees exposed to noise exceeding the OSHA Noise Action Level had the opportunity to select from only one type of hearing protection:    (a)  Propellant Charging Room (Gas Room):  On or about 9/21/16, the Line Mechanic on Line 2 had a measured 8-hour time-weighted average noise exposure of 92.7 dBA, which exceeds the OSHA Noise Action Level of 85 dBA.  The employee had the opportunity to select from only one type of hearing protection, MAX Howard Leight earplugs.    (b)  Back Room:  On or about 9/21/16, the Valver on Line 2 had a measured 8-hour time-weighted average noise exposure of 87.7 dBA, which exceeds the OSHA Noise Action Level of 85 dBA.  The employee had the opportunity to select from only one type of hearing protection, MAX Howard Leight earplugs.

29 CFR 1910.95(i)(5): The employer did not ensure proper initial fitting and supervise the correct use of all hearing protectors:    The employer did not ensure proper intial fitting and supervise the correct use of all hearing protectors for employees exposed to noise exceeding the OSHA Noise Action Level:    (a)  Propellant Charging Room (Gas Room):  On or about 9/21/16, the Line Mechanic on Line 2 had a measured 8-hour time-weighted average noise exposure of 92.7 dBA, which exceeds the OSHA Noise Action Level of 85 dBA, and the earplugs were worn incorrectly, in the outer ear.    (b)  Back Room:  On or about 9/21/16, the Valver on Line 2 had a measured 8-hour time-weighted average noise exposure of 87.7 dBA, which exceeds the OSHA Noise Action Level of 85 dBA, and the earplugs were worn incorrectly, sideways in the outer ear.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:    Propellant Charging Room (Gas Room):  On 9/21/16, employees were not protected by gloves from skin contact with hazardous materials such as, but not limited to, Household Room Fogger pesticide, one component of which is permethrin, when cans with miscrimps were being pressurized and then ruptured, with the resulting aerosol being sprayed in the direction of the employees, or when cleaning the gas heads and surrounding areas.    Shield Packaging Co.,Inc. was previously cited for a repeated violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.138(a), which was contained in OSHA inspection number 911837, citation number 2, item number 3, and was affirmed as a final order on 12/11/13, with respect to a workplace located at 50 Oxford Ave, Dudley MA 01571.
