29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) At the time of inspection, employees working in the Press Room L Line construction area were not provided fall protection when working inside the guardrail area. The floor was opened for the placement of a new structural beam, the fall to the lower level is approximately 25 feet.  b) The temporary/ mobile guardrail system put in the Press Room L line construction area was not constructed in accordance with the manufacturer's guidlines to protect employees from the fall hazard. Pins were missing in the base/ rail attachments.  c) Company manufactured and assembled guardrail systems located in the Press Room L line construction area were not assembled correctly in order to provide protection from falls for employees. The base plates were not welded as deemed necessary by the employer structural engineer.
