29 CFR 1910.1096(i)(2): All individuals working in or frequenting any portion of radiation area(s) were not instructed in the safety problems associated with exposure to radioactive materials or radiation and in precautions or devices to minimize exposure:  a)  In the Imaging Department employees exposed to fluoroscopy wore their film badges under the protective apron and were not made aware of the employer's Radiation Safety Policy, paragraph 4.4.3, Personnel Dosimetry, requiring the dosimeter to be worn at the collar outside the apron.

29 CFR 1960.8(a): 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm, in that employees were exposed to potential mutagenic, teratogenic, reproductive and other adverse health effects from exposure to hazardous drugs:  a)  On or about June 10, 2013, and at times prior thereto, employees at the facility including, but not limited to, Short Stay Unit and Urology Clinic were exposed to hazardous drugs such as Mitomycin and Eligard (Leuprolide Acetate) without them being included in a program designed to protect employees from hazardous drugs and control and reduce their exposure.    Among other methods, one feasible and acceptable means of abatement would be to:  Develop and implement a written hazardous drugs handling program as per the National Institute for Occupational Safety and Health (NIOSH) Alert, Preventing Occupational Exposures to Antineoplastic and Other Hazardous Drugs in Health Care Settings.  The program at a minimum would include the following:    (i)  A written inventory of hazardous drugs in the workplace (ii)  Labeling (iii)  Storage (iv)  Personnel issues such as exposure of pregnant workers (v)  Spill control (vi)  Detailed procedures for preparing, administering, and disposing of hazardous drugs (vii)  Training of employees with potential exposure to hazardous drugs.

29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided when necessary whenever hazards capable of causing injury and impairment were encountered:  a)  In the Urology Clinic, employee mixed and administered Eligard (Leuprolide Acetate) Antineoplastic agents without  chemotherapy gloves, disposable nonlinting and nonabsorbent gown and goggles.  PPE-covered activities include opening the outer packaging, assembling the mixing and delivery system, delivering the therapeutic drug to the patient, and disposing of all equipment used to administer drugs.    b)  On or about June 10, 2013, and at times prior thereto, employees at the facility including, but not limited to, Short Stay Unit were exposed to hazardous drugs such as Mitomycin and they were not provided and did not use chemotherapy gloves, disposable nonlinting and nonabsorbent gown and goggles.   The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 312394661, Notice number 2, Item number 1 and was affirmed as a final order on 10/19/2011, with respect to a workplace located at the Grants Pass Outpatient Clinic.
