29 CFR 1910.132(a): Protective equipment was not provided and used when necessary whenever hazards capable of causing injury and impairment were encountered: a) In the clinic, on or about June 11, 2013, and at times prior thereto, employees administered Eligard (Leuprolide Acetate), an antineoplastic agent, without chemotherapy gloves, disposable nonlinting and nonabsorbent gown and goggles. The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 312394661, Notice number 2, Item number 1 and was affirmed as a final order on 10/19/2011, with respect to a workplace located at the Grants Pass Outpatient Clinic.

The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 312392285, Notice number 1, Item number 6 and was affirmed as a final order on 11/9/2010, with respect to a workplace located at the Veterans Affairs Medical Center, Portland.
