29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about June 17, 2013, employees were exposed to struck by hazards to the head by tools or materials when not wearing a hard hat, while working and throwing pieces of lumber to the person on the pump jack scaffold above him.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: On or about June 17, 2013, employees was exposed to falls of approximately 16 feet in height to the ground level below, when working from a pump jack scaffold which did not have a guard rail system, while installing the siding to the residential home.

29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories shall be fabricated from metal plates and angles. Each pump jack bracket shall have two positive gripping mechanisms to prevent any failure or slippage. On or about June 17, 2013, employees were exposed to falls of approximately 16 feet in height to ground level below, when working from a pump jack scaffold with brackets or braces made of plywood, while installing the siding to a residential home.
