29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift: a) Spyker Contracting, Inc. on or about June 17, 2013, at the site near the corner of 5th and D streets Perrysburg, OH: The employer failed to ensure employees working from aerial lifts were adequately protected from falls. Employees working from the lifts were not clipped off to the basket while the platform was elevated. The employees were exposed to falls of up to 25 feet to the pavement below.

(a) Spyker Contracting, Inc. on or about June 17, 2013, at the site near the corner of 5th and D streets Perrysburg, OH: The employer failed to ensure employees installing a gutter system from atop the roof deck of a commercial structure were protected from a fall. The employees working in this condition were exposed to a 25 ft. fall to the pavement below.
