29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket and were sitting or climbing on the edge of the baskets guardrails or using planks, ladders, or other devices for a work position. A. Spyker Contracting, Inc. worksite located ate 190 Stadium Dr., Defiance, Ohio: On or about January 11, 2017, the employer failed to ensure employees did not stand on the guardrails of the Genie S-40 Aerial Lifts while performing steel erection related activities. The employees were exposed to a potential fall due to the weak working position. Spyker Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453 (b)(2)(v) which was contained in OSHA inspection number 912321, citation number 1, item number 1 and was affirmed as a final order on August 1, 2013, with respect to a workplace located at Perrysburg, Ohio.

Spyker Contracting, Inc. worksite located at 190 Stadium Dr., Defiance, Ohio: On or about January 11, 2017, the employer did not ensure employees personal fall arrest system was not secured (clipped-off) to the two Genie S-40 Aerial Lifts. The exposed employees were performing steel erection activities from the aforementioned aerial lifts. Spyker Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453 (b)(2)(v), which was contained in OSHA inspection number 912321, citation number 1, item number 1 and was affirmed as a final order on August 1, 2013 with respect to a workplace located at Perrysburg, Ohio.
