(b)  At Advanced Door Technologies, located at 34 North Crystal Street, East Stroudsburg, PA 18301: Employees were operating  a defective propane forklift--model number ECG25/TCM--which had tires that were visibly worn out.  Employees were exposed to powered industrial truck hazards which may result in physical injury on or about June 18, 2013  In accordance with with 29 CFR 1903.19(c), abatement certification is required for this violation.

Employees were exposed to industrial machines that could unexpectedly energize, startup, or release stored energy resulting in possible personal injury and/or death, on or about June 18, 2013.       In accordance with 29 CFR 1903.19(c), abatement certification and documentation is required for this violation.
