29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   On or about 6/19/13 in the Box Room stitching area, the following stitching machines did not provide point of operation guarding equal to one-quarter inch plus materials, exposing employees to potentially severe hand injury, when employees stepped on continuous-operation foot pedals to activate the unguarded stitching mechanism and feed boxes through the point of operation by hand:       #4 - Ideal Stitcher, Model #IS-1244, S/N A-20316 using a standard covered foot pedal      #1 - Ideal Stitcher, Model #M3-12, S/N 7575 using an Ideal wide semi-covered foot pedal      #2 - MidStates Stitcher, Model #IS-1244, S/N A-27846 using a standard covered foot pedal      #3 - Ideal Stitcher, Model #IS-1244, S/N A-47012 using a standard covered foot pedal
