29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Jobsite, 706 Duncan Avenue, Building 11, McCandless, PA:  On or about June 20, 2013, employees were working on a residential roof approximately 28 feet above the ground while setting up a tarp at the eave of the roof to protect the side of the building from damage while tearing off and disposing of the old shingles.  The pitch of the roof was 4 in 12.

29 CFR 1926.502(d): The employer used body belts as part of a personal fall arrest system and the use of body belts are not acceptable as part of a personal fall arrest system:   a) Jobsite, 706 Duncan Avenue, Building 11, McCandless, PA, On or about June 20, 2013, An employee was wearing a body belt as the only method of fall protection while working on a residential roof.  The fall hazard was approximately 28 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Jobsite, 706 Duncan Avenue, Building 11, McCandless, PA:  On or about June 20, 2013, employees were using a 32 foot aluminum extension ladder to access the roof and the ladder only extended about 18 inches above the landing surface.  The fall hazard to the employees was approximately 28 feet to the ground.
