29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:    a) Jobsite, 3315 Cargill Street, Pittsburgh, PA:  On or about March 23, 2018, 6 employees were using a 32 foot, Werner extension ladder that had a crack in one of the rungs and several others rungs were bent.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  a) Jobsite, 3315 Cargill Street, Pittsburgh, PA:  On or about March 23, 2018, ladders were not inspected for visible defects that included one cracked rung and several bent rungs on the extension ladder being used to access the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Jobsite, 3315 Cargill Street, Pittsburgh, PA:  On or about March 23, 2108, six employees were tearing off shingles on a roof at heights of 23-40 feet above the ground without fall protection.    Battist Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 912608, citation number 1, item number 1a and was affirmed as a final order on August 26, 2013, with respect to a workplace located at 706 Duncan Avenue, Building 11, Pittsburgh, PA.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite, 3315 Cargill Street, Pittsburgh, PA:  six employees were tearing off shingles on a roof and without utilizing fall protection and had not been trained to recognize the hazards of falling and how to protect themselves from it.   Battist Contruction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 912608, citation number 1, item number 2 and was affirmed as a final order on August 26, 2013, with respect to a workplace located at 706 Duncan Avenue, Building 11, Pittsburgh, PA.
