29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Showroom: On or about June 11, 2013, an employee laying out drywalls with a subcontractor was not wearing a hard hat. The employee was exposed to the hazard of being struck on the head by construction materials.

29 CFR 1926.502(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guardrail system and the walking/working level: Show room mezzanine: On or about June 12, 2013, an employee and subcontractor employees preparing to construct drywalls on the mezzanine were along a section of the mezzanine open side that did not have a midrail in its guardrail system. The employee and a subcontractor employee were kneeling and were below the top rail. This exposed the employees to the hazard of falling over 6 feet.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge: Showroom mezzanine stairway: On or about June 12, 2013, an employee and subcontactor employees preparing to construct the mezzanine drywalls used the mezzanine stairway. The stairway did not have handrails or stairrails on its open sides exposing the employees to the hazard of falling.
