29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, strucure, personnel hoist, or similar surface was not used: Location: West side of Farrell Backland Insurance building Employees were exposed to fall hazards while climbing the ladders supporting the ladder jacks to access the platform where no other access was provided.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: Location: West side of Farrell Backland Insurance building Employees were exposed to a fall hazards of up to 12-feet to the ground below while working from the platform of a ladder jack scaffold with no fall protection.
