29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction: a) Steel Division Walking surface: On or about 06/21/2013, an approximately 1-2 foot deep drainage trench, that contained a 12" x 6" hole in one portion of its length; was not guarded by a floor hole cover of standard strength and construction to protect employees from fall/tripping hazards while traversing the area in order to complete job duties.
