29 CFR 1910.133(a)(3):  The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:        a) Throughout the facility: On or about 01/10/2014, the employer did not provide employees who wore prescription lenses while engaged in stone grinding and/or cutting work while using equipment such as, but limited to the DeWalt 4.5" Grinding tool; with American National Standards Institute (ANSI) approved prescription safety glasses with sideshields nor eye protection that could be worn over the prescription eye lenses at the facility.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:      The employer did not have a written respiratory protection program for employees who wore 3M 6000 Series Half Mask Respirators with P100 cartridges with the following exposures:      a) Segmental Patching Area: On or about March 20, 2014; an employee performing sanding and cutting of stone using  tools such as, but not limited to a Bosch (M#: 11263EVS) Hammer Drill and a DEWALT D28110 4.5 Small Angle grinder; was exposed to Respirable Silica Dust containing 12% silica. The employee's sampled exposure level was 2.37 mg/m3, which was 3.32 times the calculated permissible exposure level of 0.7143 mg/m3.  Air sampling was conducted for 128 minutes. An exposure level of Zero (0) mg/m3 was assumed for the unsampled time.      b) Segmental Patching Area: On or about March 20, 2014; an employee performing sanding and grinding of stone using tools such as, but not limited to a DEWALT D28110 4.5 Small Angle grinder and a portable sander to remove imperfections from a precast concrete form; was exposed to Respirable Silica Dust containing 28% silica. The employee's sampled exposure level was 15.75 mg/m3, which was 47.3 times the calculated permissible exposure level of 0.7143 mg/m3.  Air sampling was conducted for 117 minutes. An exposure level of Zero (0) mg/m3 was assumed for the unsampled time.
