29 CFR 1910.119(j)(4)(iv): The documentation of the inspection or test that had been performed on process equipment to maintain its mechanical integrity did not identify the date of the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test, and the results of the inspection or test. The employer does not ensure results from inspections and tests performed on process equipment are documented. The violation occurred in the Ammonia #1 and #2 units on or about June 20, 2013 and at times prior thereto where the employer failed to ensure results from inspections and tests performed on identified safeguards were documented. Identified safeguards include but are not limited to: a. EFT-102 A/B and associated logic b. TI-46/47 and associated logic c. PIC-1A and associated logic d. EFT-3 A/B and logic e. EF3LL EF-102LL and associated logic and controls f. ETI-14 and 106DHH logic g. EPI-160 A/B/C and LF160HH logic h. EPI-181 Alarms and Trips EPT-19 A/B and EF102LL logic i. EPT 8 A/B and V-8A logic This condition exposed employees to fire, explosion, and/or inhalation hazards. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure results from inspections and tests performed on process equipment are documented with the date, serial number or equipment identifier, and a description of the of the inspection or test.
