29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. On or about January 22, 2015, at the Urea #3 Unit, I&E technicians were attempting to clear a plugged vent and tubing routed to an ammonia pressure transmitter which was capable of dispersing liquid ammonia. These employees were not adequately protected by a full body covering and proper eye protection.
