Sensus USA Inc.; Iriedite 14 - On or about 6/24/13, employees were utilizing a man basket on an order picker (Crown Model #30SP3611) without manufacturer's approval.  The man basket was used for activities including, but not limited to, to lift employees to change light bulbs in the facility.  Furthermore, the company had not requested a new plate for the order picker to identify the weight of the man basket and the change in capacity that it would create.
