29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval and the forklift plate was not changed to reflect the attachment: A.) Sensus USA Inc.; Iriedite 14 - On or about 6/24/13, employees were utilizing a man basket on an order picker (Crown Model #30SP3611) without manufacturer's approval. The man basket was used for activities including, but not limited to, to lift employees to change light bulbs in the facility. Furthermore, the company had not requested a new plate for the order picker to identify the weight of the man basket and the change in capacity that it would create.

Sensus USA Inc.; machining department - Cincinnati tool and cutter grinder #WD9013E --On or about 6/24/13, the grinding wheel guard was not adjusted properly in that the guard was not positioned close enough to the wheel.
