29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Second floor, new residential construction located at 1902 Sadler Drive, Smyrna, GA 30080: Employees snapping chalk-lines and laying out second floor framing at heights of approximately 15-feet, 3-inches and 21-feet, 7-inches were exposed to falls to lower levels when a fall protection system was not provided.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) First floor, new residential construction located at 1902 Sadler Drive, Smyrna, GA 30080: Employees were exposed to falls of approximately 11-feet, 2-inches while using a Werner 8-foot step ladder to access the second floor working surface located at heights of approximately 11-feet, 2-inches from the first floor. The ladder used to access the second floor did not extend at least 3 feet past the upper landing surface.
