29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) Roof, new residential construction located at 13815 Bethany Oaks Pointe, Milton, GA 30004: Employees operating pneumatic Hitachi (Model - NR83A2S1) 3-1/4-inch Framing Nailers and Hitachi (Model - NV65AH) Coil Nail Guns were not wearing protective eyewear. Employees were exposed to struck-by hazards which included plastic and wood debris and fasteners as safety glasses and/or goggles were not worn while operating these tools at 120PSI.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  (a) Rear side, new residential construction located at 13815 Bethany Oaks Pointe, Milton, GA 30004: Employees working on a residential roof at heights of approximately 21-feet, 6-inches were not protected from falls while working over uncapped rebar which was protruding out of the concrete foundation directly below the roof. Employees working over the 9-inch unguarded rebar were exposed to impalement hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Roof, new residential construction located at 13815 Bethany Oaks Pointe, Milton, GA 30004: Employees installing 1/2-inch plywood sheathing on a 10/12 pitched roof at heights of approximately 21-feet, 6-inches were not provided with a fall protection system.   Genaro Bueno Dba La Providencia Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 913375, citation number 1, item number 1 and was affirmed as a final order on 04/17/2014, with respect to a workplace located at 1902 Sadler Drive, Smyrna, GA 30080.
