29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.      Location: 3965 Virginia Ave, Cincinnati, OH 45227    On or about 24 June, 2013    a) Employer did no establish a program to protect employees from unexpected energizing or startup of labeling, bottle filling and sorting machines while maintenance operations were being performed.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.      Location: 3965 Virginia Ave, Cincinnati, OH 45227    On or about 6 June, 2013    a) Employees were exposed to electrical conductors entering the cutout box for a drive motor on a bottle conveyor system that were not protected from abrasion in that the outer insulation of the wires were not covered with insulation.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
