29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  Atlanta Insulation & Contracting Services, Inc. - on or about June 25, 2013, employees were required to wear supplied air respirators when applying foam insulation.  The employer had not developed and implemented a written Respiratory Protection program.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:   Atlanta Insulation & Contracting Services, Inc. - on or about June 25, 2013, employees were required to wear supplied air respirators when applying foam insulation.  The employer did perform medical evaluations to ensure employees were capable of using respirators without creating a hazard.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  Atlanta Insulation & Contracting Services, Inc. - on or about June 25, 2013, an employee used a filtering facepiece respirator when loading fiberglass insulation onto trucks.  The employee was not provided with the information in Appendix D of the Respiratory Protection standard.
