29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Tool Room, on or about 6/25/13: The Bridgeport Mill was not provided with a guard to protect the operator from contact with the rotating chuck and tool during operation. b) Too Room, on or about 6/25/13: The Chevalier Mill was not provided with a guard to protect the operator from contact with the rotating chuck and tool during operation. c) EDM Department, on or about 6/25/13: Porter-cable Sander was not provided with nip point guarding on both sides of the belt where the belt runs onto the roller. d) Lathe Department, on or 6/25/13: CNC Lathes, such as but not limited to, Hardinge #320, 321 and 322, had the door interlocks disabled.
