See American National Standard Safety Code for Powered Industrial Trucks, B56.1-1969, which is incorporated by reference as specified in Sec. 1910.6. a) On or about June 7, 2013, a 4"x4"x5' wood crosser fell from a load being elevated and came through the overhead guard striking the operator in the head and shoulder. The employee suffered injuries to the head and shoulder. ABATEMENT CERTIFICATION REQUIRED
