29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe-boards: a) On or about August 26, 2015 employee(s) climbing on top of the wood stack from the stacker line platform were not protected from falling onto metal rollers and/or the floor below from a distance of greater than 4 feet. b) On or about August 26, 2015 employee(s) leaning over a guardrail 33 inches high on the stacker line platform were not protected on the sides of the wood stack from falling down onto metal rollers and/or the floor below. c) On or about August 26, 2015 an employee was not protected from a fall hazard through an opening 30 inches wide by 40.5 inches high next to the rip saw #1 station near the excess wood stacking area.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a) On or about August 26, 2015 in-running nip points, created by an unguarded chain and sprocket powering the conveyor table on the stacker line platform, was not enclosed. b) On or about August 26, 2015 the in-running nip points created by six unguarded chains and sprockets underneath the stacker line platform were not enclosed. c) On or about August 26, 2015 in-running nip points created by three unguarded chains and sprockets on the re-alignment conveyor at rip saw #1, were not enclosed. The Aacer Acquisitions, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(f)(3) which was contained in OSHA inspection number 315288548, citation number 1, item number 1(b) and was affirmed as a final order on August 23, 2011, with respect to a workplace located at 970 Ogden Road, Peshtigo WI 54157-1727.
