29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: Location: 12300 St. Charles Rock Road, Bridgeton, MO 63044 On or about June 21, 2013 a Hek Manufacturing MS 5000 mast climbing platform used by employees at the project was not constructed in accordance with an engineer's approved design or Hek's Manufacturing User's Manual for the MS 5000 Mast Climbing Work Platform. The mast climbing platform was elevated to approximately 14 feet above ground when the three left extension platforms collapsed.

29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed such that the components fit together with force and the scaffold's structural integrity was not maintained by the user: Location: 12300 St. Charles Rock Road, Bridgeton, MO 63044 On or about June 21, 2013 a Hek Manufacturing mast climbing platform, model HEK MS 5000, was erected with one extension MS platform element of 175 (5 ft. 9 in. length), one extension MS platform of 150 (4 ft. 11 in. length), and one unknown extension platform element (approx. length 6 ft. 6 in.) attached to the left of the base working platform unit. The three left extension platforms collapsed unexpectedly as the unknown extension platform failed at the left side connection point to the main MS 5000 platform unit.
