29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:    a)There was an employee observed working from a 9 1/2" wide scaffold plank work platfrom without conventional fall protection exposed to a fall potential in excess of 9' supported between an elevated JCB rough terrain forklift mast/fork with attached loaded pallett and an adjacent self-supported tubular welded frame scaffold system.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178.      a) An employee/operator elevated a JCB rough terrain forkllift mast/forks and taaached loded p[allet with an unprotected employee occupied single scaffold plank placed to an adjacent self-supported tubular welded frame scaffold system. The forklift operator performing the duties was not trained on the workplace hazards associated with forklift operation and the requirements of this regulation.
