OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to "stuck by" hazards associated with machinery setup.     (a) Spendrup Fan Company @ 2768 C ½ Road, Grand Junction, CO: On and before June 25th, 2013, the employer did not ensure the operator of the Knuth R60 boring machine had been instructed and understood how to safely set up and align the machine with the material being drilled (fan hubs).  The operator engaged in setting up the boring machine to drill multiple 5 inch holes through fan hubs was observed hand-gripping the rotating spindle to slow its rotation. The condition exposed the employee to hand/knuckle injuries associated with the rotating spindle.     Among other feasible and acceptable methods to abate this hazard is to follow the American National Standards Institute (ANSI) B11.8-2001 document titled "Safety Requirements for Manual Milling, Drilling and Boring Machines with or without Automatic Control". Paragraph 4.3(i) states the following. "Never slow or stop the spindle or workpiece by hand-gripping of the spindle, workholding device or the workpiece; likewise, never use an improvised breaking device (e.g., pry-bar)".

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:   (a) 29 CFR 1910.157(g)(1): Spendrup Fan Company, 2768 C ½ Road, Grand Junction, CO:  On and before June 24th, 2013, the employer had not developed and implemented an educational program to familiarize employees working in the fabrication areas with the general principles of fire extinguisher use and hazards involved with incipient stage fires.  Abatement Note: Per OSHA regulation 1910.157(g)(2), the employer shall provide the above training upon initial assignment to the designated employee(s) and at least annually thereafter.  Abatement Note: OSHA Subpart Q, section 1010.252(a)(2)(iii) titled Welding, Cutting and Brazing require all fire extinguishers to be maintained in a state of rediness for instant use.
